We also realize how busy families are today, and we want to make keeping yours healthy as easy and streamlined as possible. That is why our practice is excited to announce that we now also accept pediatric patients.
With a range of specialties that run from pediatrics to geriatrics, our physicians and skilled medical professionals strive to always provide outstanding care to achieve the highest possible patient satisfaction.

Please explain the reason for your visit. This allows us to schedule the appropriate time needed to address your healthcare concerns. For your visit, please arrive 15 minutes prior to your appointment time so that we may obtain the information required for establishing your care.
• Each visit bring your insurance card with you as well as a list of your current medications. • Please inform the front desk staff of any change in name, address, telephone number, employment or insurance. • Always inform your physician of any changes in medical history.
If you have an urgent need to be seen, please call as soon as possible, and we will try to accommodate you. We do our best to stay on schedule, but emergencies may sometimes cause unexpected delays.
Please extend these courtesies to others in our office: no smoking, no perfumes. Please turn off your cell phone when in the exam room. As a courtesy to you, our automated reminder system will call and remind you of your appointment 2 business days prior to your appointment.

Payment is expected at the time of service. We accept cash, check, VISA and Mastercard. If we are contracted with your insurance company, you will need to pay your co-pay at the time of your visit. This amount will be indicated on your insurance card. We are contractually bound with the insurance companies to collect this at the time of service. We will then file your insurance claim. However, payment for the account remains your responsibility.
If you have insurance that we are not contracted with, you will be asked to make full payment, and file your own insurance claim. Patients scheduling procedures will be required to pay a percentage at the time of service.
Please maintain your account with our office in good standing. Should your account become delinquent, your account may be turned over to an outside collection agency. Should this become necessary, you will be responsible for all additional expenses incurred to collect this outstanding debt and you will be discharged from the practice.
Your medical records are strictly confidential. The Health Insurance Portability and Accountability Act (HIPAA) restricts us from releasing any information without your written permission.
There may be times when you request that we provide copies of our records to you or other entities. You will be billed for this service.
You will receive prompt notification of any results that require immediate attention, otherwise please allow 7-10 days for test completion. For normal test results requiring minimal follow-up, we will notify you by mail. Otherwise our staff will call you with the results and the physician’s instructions.
If at any time you have additional questions, feel free to make an appointment with the doctor. If you do not receive notification of your results in the above time frame, please call the office to inquire about them.

Prescription refills will only be authorized during normal office hours. Allow 48 hours for this process to be completed. Please evaluate your medication supply prior to your office visits and request refills at your visit. For your convenience, we have a Prescription Refill Line, it is option # 1 when you call 812-372-8281. You will need to leave your name, date of birth, medication to be refilled by name, strength, amount (90 day, 30 day), instructions for taking medicine, physician, pharmacy and your phone number for any questions. This line is checked periodically during the day by our clinical staff. Please allow 48 hours for request to be completed.
Patients must have been seen by a health care provider in the prior six months. If you have not seen your provider within the recommended time frame for follow-up, you will need an appointment before refills can be authorized. It is our policy that no controlled substances will be called in after hours or on weekends and we will not phone in any type of narcotic pain medication to pharmacies.
